Introduction

Oral and maxillofacial injuries commonly result from occupational accidents, sports trauma, traffic collisions, and other daily life incidents. This region features a rich vascular network, close proximity to the brain and neck, and serves as the entrance to both the respiratory and digestive tracts. It consists of multiple bones and sinus cavities, with teeth embedded in the jawbones and the tongue housed within the oral cavity. The face also contains mimetic muscles and the facial nerve, along with the temporomandibular joint and salivary glands—all essential for expression, speech, mastication, swallowing, and respiration.

Fixation following the reduction of maxillofacial fractures is a critical phase of treatment. Common fixation approaches include single-jaw arch splinting, intermaxillary fixation, interosseous wiring, miniplate or microplate osteosynthesis, and craniomandibular fixation. Additional methods such as circummandibular wiring and compression plating may also be employed.

• Single-Jaw Arch Splint Fixation: A pre-formed or custom-shaped arch splint (2 mm aluminum wire or ready-made hook-equipped splint) is ligated to the teeth on both sides of the fracture using fine stainless steel wires. This approach is suitable for non-displaced or minimally displaced fractures, such as midface linear fractures or localized alveolar fractures.
• Intermaxillary Fixation: Hooked arch splints are applied to both the upper and lower dental arches. Elastic bands or wires are used to maintain the jaws in normal occlusal position. This reliable method suits various mandibular fractures, ensuring proper healing and functional recovery. Drawbacks include limited oral intake and challenges in maintaining oral hygiene, requiring attentive nursing care.
• Interosseous Wiring Fixation: During open reduction, holes are drilled into the fracture ends, which are then secured with stainless steel wires. This offers reliable stabilization and is also applicable in pediatric jaw fractures or edentulous cases.
• Miniplate or Microplate Fixation: After open reduction, a suitably shaped miniplate (for the mandible) or microplate (for the maxilla) is placed across the fracture line. Special screws secure the plate to the bone cortex, providing rigid internal fixation.
• Cranio-Maxillofacial Fixation: In cases of maxillary transverse fracture where mandibular support is insufficient, the skull can serve as an anchor. An arch bar is attached to the maxillary teeth, connected via wires that pass through the zygomatic soft tissues to a headcap or external frame. This is often combined with intermaxillary fixation.

The duration of fixation depends on the patient's injury, age, and overall condition. Typically, maxillary fractures are immobilized for 3–4 weeks, and mandibular fractures for 4–8 weeks. A dynamic-static approach can shorten fixation time: after 2–3 weeks of rigid fixation, elastics may be removed during meals to allow guided motion. With rigid internal fixation using miniplates or microplates, early functional exercise can be initiated to support healing.
